We are seeking a Support Coordinator to join our team in a three month fixed term, full time role.
The CHOICES Support Coordinator will engage NDIS Participants and their families, who have support coordination in their plans, in the development of their CHOICES Service Agreement and support arrangements to ensure that their needs and wishes are addressed, and NDIS goals achieved. The Support Coordinator will promote individuality, positive outcomes, choice, and control.
