
The Children’s Behavioral Health Home (BHH) Coordinator / Targeted Case Manager is responsible for overseeing and coordinating comprehensive care services for children and families with behavioral health needs.
This full-time role blends direct client support with care coordination, documentation, advocacy, and collaboration with multidisciplinary teams to ensure high-quality, person-centered care.
The ideal candidate is compassionate, organized, and skilled at engaging children and families — including those who may be hard to reach or hard to engage — while navigating complex systems of care.
Requirements
· The position requires a bachelor’s degree with a specialization in psychology, mental health, human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing, or a closely related field.
· OR have a bachelor’s degree from an accredited four-year educational institution in an unrelated field and at least one (1) year of full-time equivalent relevant human services experience.
