Job Duties:

  • The CFTM Facilitator is responsible for facilitating CFTM's for children who are at risk of removal from their home, who are experiencing foster care instability, and at other identified opportunities during a child's involvement with the agency. The primary purpose of this role is to engage families and partners in collaborative decision-making to identify supports and strategies that prevent foster care entry, stabilize family or foster care placement, and promote overall child safety, stability, and well-being.
  • Provides Child and Family Team Meeting services through preparation, planning, and facilitating Removal Prevention CFTM's for non-custodial cases or to prevent further CWS involvement (i.e. child from entering foster care), placement CFTM's for children experiencing foster care instability, and other CFTM's as requested such as Initial, 25-Day, Special Call, etc. This may also include on-call facilitation.

  • Engages families in identifying kin and natural supports to build the Child and Family Team. This may include utilization of family finding tools such as Genograms, Ecomaps, Connectedness Charts, and CLEAR or Seneca Searches. Ensure that families have supports and kin present at the table for the Child and Family Team Meeting.
  • Documents CFTM's accurately in CAPSS and other mechanisms.
  • Increases community and provider network awareness of DSS family engagement practices such as child and family teaming's to prevent foster care entry and child welfare involvement.

  • Ensures that Personal Identifying Information (PII) is properly used, accessed, gathered, shared and disposed. Protects the agency networks and applications by safeguarding systems, equipment and data.
  • Performs other duties as assigned.

Examples of duties

A Bachelor's Degree in Social Work, Behavioral Science or Social Science.

A Bachelor's Degree in any other related field and one (1) year of professional experience in human services or social service programs

A Master's Degree or higher may be substituted for required work experience.
