Job Description

Provides direct support services and spiritual care to patients and their families with compassion, caring, and respect.

Responsibilities

Patient/Family Support and Spiritual Care

    • Assesses the spiritual needs of patients and families.
    • Demonstrates effectiveness in the spiritual care of individuals regardless of their belief system, values or cultural factors.
    • Develops and coordinates communication with local clergy.
    • Works with staff, local clergy, and community groups to enhance their sensitivity to the spiritual concerns of patients/families.
    • Performs other duties as assigned by supervisor.
    • Provides supportive care in times of grief, crisis and end-of-life.
    • Provides emotional/spiritual support to patients, families, and staff.

Patient Family Centered Care

    • Gathers and evaluates relevant data pertinent to the patient’s situation and/or bio-psycho-social-spiritual/religious health.
    • Develops and implements a plan of care to promote patient well-being and continuity of care.
    • Enters information into the patient’s medical record that is relevant to the patient’s medical, psycho-social, and spiritual/religious goals of care.
    • Incorporates patient, family as defined by the patient and other team members when planning care and service delivery.
    • Demonstrates an attitude that creates an atmosphere of respect for human differences, beliefs, values and the uniqueness of each person encountered.
    • Partners with patients and families to allow for effective decisions and participation in care.
    • Shares complete and unbiased information with patients and families, with patients' consent.

Qualifications

  • Master of Divinity or Master of Theology
  • Clinical Pastoral Care Certification from an accredited institution including theological studies or equivalent in experience, skills, and education
