Meet People Where They Are — In Every Sense

Transitions Hospice Chaplains do something remarkable — they bring comfort, connection, and spiritual support into people's homes at a time when families need it most. If you're someone who listens deeply, honors all beliefs, and finds meaning in serving others, this role was made for you.

We don't define spirituality narrowly. Our approach is non-denominational, deeply human, and guided entirely by the needs of the person in front of you. If you're someone who can hold space for grief, uncertainty, and meaning across any faith tradition or none at all — we want to hear from you.

What You'll Do

As a Transitions Hospice Chaplain, you'll be the spiritual anchor of the interdisciplinary care team — connecting patients and families to the support they need, in the way that resonates most with them. Your responsibilities include:

  • Assessing the spiritual needs of patients, families, and caregivers with sensitivity and timeliness
  • Providing non-denominational spiritual care directly, or coordinating with preferred spiritual advisors and clergy
  • Developing, documenting, and continuously refining individualized spiritual care plans with the interdisciplinary team
  • Educating and consulting with care team members on spiritual and emotional support best practices
  • Building relationships with local faith communities to expand the network of spiritual support available to patients
  • Conducting funeral, memorial, and bereavement services as requested — including annual services for Transitions families
  • Participating in on-call duties on evenings and weekends as needed
  • Maintaining accurate records in compliance with local, state, and federal requirements
  • Showing up as a positive, motivating presence for colleagues and the broader care team

Benefits We Offer

  • Competitive pay
  • Company provided equipment
  • Paid Time Off

Why Choose Transitions Hospice?

  • Strong interpersonal skills and the ability to collaborate within a clinical interdisciplinary team
