PurposeCare is seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join our team as a Change in Condition (CIC) Team Lead
In this role, you'll serve as a vital connection between our Home Care and Home Health teams by reviewing changes in patient conditions, identifying opportunities for skilled home health services, and coordinating care to ensure patients receive the right services at the right time. Working closely with the Clinical Liaison and Home Health Intake Coordinator, you'll help improve patient outcomes while supporting seamless transitions of care.
Review Change in Condition (CIC) forms submitted by Home Health Aides.
Identify patients who may benefit from skilled home health services.
Coordinate referrals with the Clinical Liaison and Home Health Intake Coordinator.
Track and follow up on hospitalized or skilled nursing facility patients returning home.
Educate patients and families about available home health services and continuity of care.
Conduct follow-up calls and visits to assess ongoing patient needs.
Complete caregiver skills checks during orientation.
Educate caregivers on recognizing changes in patient condition and the CIC process.
Maintain accurate documentation and ensure timely communication across care teams.
We're looking for someone who has:
Current Licensed Practical Nurse (LPN) license in the state of practice
Graduate of an accredited nursing program
Home health or healthcare experience preferred
Excellent communication and relationship-building skills
Strong critical thinking, assessment, and problem-solving abilities
Ability to work independently while collaborating with interdisciplinary teams
Strong organizational and documentation skills
Passionate about improving patient outcomes
Organized and detail-oriented
Comfortable educating patients, families, and caregivers
Strong clinical judgment
Excellent time management and follow-through
Team player with a patient-first mindset
