40Reports to the Social Work Manager, the Certified Social Worker (CSW) is a member of the Care Coordination – Integrative Care team and located in the primary care office to provide integrative mental health care services to patients of the care practice. The CSW is primarily responsible for face to face comprehensive biopsychosocial assessments and collaborate with the patient’s primary care physician and other medical staff to determine diagnosis and treatment through brief therapy and strive for a healthy mental health outcome.
The nature of this practice will consist of working with children, adolescent, adult and geriatric patients who present with emotional disorders and mental illness in a primary care setting. The CSW will be provided in-house individual and group supervision until the CSW obtains a clinical licensure of a LCSW.

EDUCATION:
Minimum: MSW degree accredited program, Certified Social Worker (CSW)
LICENSES AND CERTIFICATIONS:
Minimum: Class D driver’s license
Certified Social Worker (CSW – KY; with preference for additional licensure in OHIO/IN as LSW)
YEARS OF EXPERIENCE:
0-2 years of social work experience in mental health; prefer medical social work experience
