As a Certified Nurse Assistant with Touchstone Health's Hospice team in San Antonio, you'll provide hands-on, compassionate care to hospice patients in the comfort of their own homes. You'll support activities of daily living, monitor vital signs, and offer the kind of steady, personal presence that helps patients maintain dignity and comfort at one of life's most meaningful stages. This role is a natural fit for someone who wants to build real relationships with the patients and families they care for, not just check tasks off a list.
Touchstone Health's Hospice team walks alongside patients and families during one of life's most difficult transitions, and as a CNA, you'll be a vital part of that care. We offer medical, dental, and vision coverage, PTO, a 401(k), mileage reimbursement, flexible scheduling, and competitive compensation growth. You'll also have access to an Employee Assistance Program, recognition programs, and a referral bonus of up to $2,000. If you're someone who shows up with patience and care for the people who need it most, we'd be honored to have you on our team.

For more than 100 years, CareM and its predecessors have been building, operating, and managing healthcare organizations across the continuum of care. Five generations of leadership, from sanitariums in the Deep South to a modern clinically integrated network, have shaped how we think about care, operations, and the people who deliver both. Over the years, CareM has provided managed services to private ownership groups, not-for-profits, municipalities, and Native American tribes.
Our mission is simple: to bring out the courage in others. We exist to help people stay home, through skilled care, hospice, palliative care, and physician services, and to build the infrastructure that lets clinical teams do that work well.
We don't deliver care directly. We make it possible. CareM provides strategic guidance, operational support, and centralized services to our managed entities, while coordinating value-based care integration across a broader provider network.
We've worked with organizations at every stage, from startups to established nonprofits to joint ventures, and the lesson that has held across all of them is that people deliver care, and people need systems that support them.
