Position: Certified Home Health Aide (CHHA)
Locations: Middlesex, Union, Somerset County

Starting Wage: Based on experience

What You'll Do:

As a Certified Home Health Aide at Grace Healthcare Services, every day is an opportunity to make a difference. You'll provide:

• Personal Care: Assist patients with bathing, grooming, dressing, skincare, and mobility support.
• Meal Preparation: Create nutritious meals tailored to individual dietary needs.
• Home Management: Maintain clean, safe, and comfortable patient living spaces.
• Documentation: Use Celltrak to accurately record patient care activities and ensure smooth communication.
• Team Collaboration: Work closely with RN Case Managers to monitor and report changes in patient conditions.

What You Need:

• NJ Certified Home Health Aide (CHHA) License: Must be in good standing.
• Communication Skills: Ability to read, write, and fluently communicate in English.
• Physical Readiness: Capable of lifting, pushing, and pulling up to 50 lbs
• Flexibility: Willingness to travel within your assigned territory, to bring care to those who need it.
