The Certified Coder analyzes clinical and demographic information from the health record to assign appropriate ICD-10-CM and CPT/HCPCS codes in accordance with established guidelines and regulatory requirements. This role supports accurate and compliant coding of outpatient medical encounters, contributing to data integrity, appropriate reimbursement, and overall performance of the health system.

Performs review and analysis of medical records to assign diagnosis and procedure codes for outpatient encounters. Applies ICD-10-CM, CPT, and HCPCS coding guidelines in accordance with regulatory and organizational standards. Identifies documentation gaps and initiates queries for clarification. Resolves routine coding edits and initiates inquiries to ensure accurate and compliant code assignment. Maintains productivity and accuracy standards while managing assigned work queues and supports timely resolution of coding related questions.

  • Works cooperatively with coding team members and internal partners to complete work accurately and efficiently
  • Clearly communicates documentation needs and coding questions to appropriate stakeholders
  • Uses coding tools and references to resolve routine issues and escalates when needed
  • Completes assigned coding work accurately and within established timelines
  • Follows established workflows and identifies basic opportunities for efficiency
  • Supports accurate coding outcomes that contribute to reimbursement and data integrity
  • Adapts to changes in coding guidelines, tools, and payer requirements

REQUIRED QUALIFICATIONS

EDUCATION

  • High School Diploma or GED

LICENSE/CERTIFICATIONS:

  • AHIMA-CCA (Certified Coding Associate) OR AHIMA-RHIA (Registered Health Information Administrator) OR AHIMA-RHIT (Registered Health Information Technician) OR AHIMA-CCS (Certified Coding Specialist) OR AAPC-CPC (Certified Professional Coder) OR AAPC-CPC-A (Certified Professional Coder Apprentice), AAPC-CIC (Certified Inpatient Coder), AAPPC-COC (Certified Outpatient Coder)

SKILLS:

  • Day-to-day work requires: applying ICD-10-CM and CPT/HCPCS coding for low-complexity outpatient encounters; utilizing coding guidelines and payer rules to ensure accurate and compliant code assignment; reviewing documentation, identifying gaps, and initiating clarification as needed; resolving routine coding edits; and maintaining productivity and accuracy standards within assigned work queues.
