Central Intake Coordinator

Location: Dayton, OH (Corporate Office – On-site)
Schedule: Monday–Friday, Full-Time

We are seeking a motivated and detail-oriented Central Intake Coordinator to join our corporate team at a growing senior care organization specializing in Skilled Nursing, Memory Care, and Assisted Living

This is a fast-paced, high-impact role responsible for managing and streamlining the admissions process across multiple facilities.

What You’ll Do

In this role, you will:

• Manage and coordinate incoming referrals from hospitals, case managers, and families
• Review referrals for clinical appropriateness, payer source, and bed availability
• Route and place referrals to the appropriate facility within the organization
• Communicate with hospital partners and facility teams to ensure timely admissions
• Verify insurance coverage (Medicare, Medicaid, Managed Care, Private Pay)
• Support census development and help maximize occupancy across buildings
• Ensure all admission documentation is complete and compliant
• Work closely with facility Admissions teams, DONs, and Corporate leadership
• Help streamline and improve the overall intake and admissions process

What We’re Looking For

• Experience in Senior Care, Healthcare Admissions, or Hospital Case Management preferred
• Strong understanding of Medicare, Medicaid, and Managed Care (preferred)
• Excellent communication and follow-up skills
• Ability to work in a fast-paced, high-volume environment
• Strong attention to detail and organizational skills
• EMR experience (PointClickCare or similar preferred)
• Team player with a sense of urgency and accountability
