JOB TITLE: Central Bed Coordinator

SCHEDULE: Monday - Friday7:30am-3:30pm, 3:30pm11:30 pm
The Administrative Bed Coordinator ensures appropriate utilization of hospital beds or beds in a LTC facility through frequent collaboration with the professional staff in facilitating more timely transfers, establishing priority inpatient admissions and appropriate placement of patients. Supports hospital Patient Flow by promoting and sustaining an environment dedicated to the safe, efficient placement of all patients moving through the System.
RESPONSIBILITIES
Responsible for monitoring and managing patient bed needs as it pertains to admitted patient placement at Rochester General Hospital/ Newark Wayne Hospital or Demay Living Center
Tracking all new admissions for the procurement and communication of bed assignment for the adult/pediatric - medical/ surgical population from various entry points such as ED, OR/PACU, Spec Procedures/Endoscopy, Direct Admits (Clinics, Homes, MD Offices, Nursing Homes), Cardiac Cath Lab external locations
Real Time management of incoming Admission demand to determine bed resources needed
Real Time management of discharges, creating open bed resources, escalating when demand exceeds standard bed capacity or other barriers are encountered
Real Time escalations for Bed Clean priority changes to Environmental Services
Rounds on Main OR PACU and ASC OR validating patient movement and patient Flow Round on units as needed to gain insight on daily bed issues
Monitor key Unit resources and escalate when capacity triggers are met. Monitoring isolation bed needs and escalating to Infection Prevention Team if co-habitations might be needed
Collaborate with other disciplines and departments to maximize the use of beds and facilitate patient movement
Assigning appropriate beds for existing admitted patients from Unit to Unit based on patient needs as identified by providers and nursing staff
Resource to Units when Care Connect issues arise during patient movements
Communicates with Administrative Clinical Coordinator on daily basis and more often as needed regarding bed availability, census status, overcapacity triggers and escalations at RGH and NWH
Utilizing the RGHS EMR Care Connect – works in collaboration with ED, Unit Clinical Professionals to ensure that patients are admitted to the appropriate unit and are at the appropriate level of care
Data maintenance of various databases
Responsible for correspondence as directed by leadership.
Assist in managing issues/complaints

PREFERRED QUALIFICATIONS:
High School Diploma or 2 year degree with minimum of 5 years administrative office experience in health care
Excellent oral and written communication and interpersonal skills
Strong organizational and problem solving skills
High level of proficiency with PC, software applications, and reporting tools.
May need previously experience in CMS, Medicaid/Medicare billing, and other regulatory processes
Bilingual
