The Company

We are seeking an experienced and patient-focused Registered or Enrolled Nurse to join our Deception Bay Medical Centre on a part-time basis, specialising in Chronic Disease Management and preventative healthcare.
The Role
As our CDM Nurse, you will work closely with GPs, nurses, allied health professionals and patients to coordinate personalised care for people living with chronic health conditions.
You will empower patients to take greater ownership of their health through education, goal setting, ongoing monitoring and practical support.
Key Responsibilities
• Prepare GP Management Plans and Team Care Arrangements
• Complete patient histories, biometrics and chronic disease assessments
• Work with patients to establish practical SMART health goals
• Develop action plans and coach patients through sustainable health-behaviour changes
• Coordinate referrals to allied health professionals and specialists
• Conduct preventative health assessments, including:
◦ Heart health assessments
◦ Diabetes risk assessments
◦ 45–49-year-old health assessments
◦ Over-75 health assessments
◦ Aboriginal and Torres Strait Islander health assessments
◦ Intellectual disability, veteran and refugee health assessments
• Provide ongoing monitoring and support to eligible patients
• Perform clinical services such as ECGs, spirometry, urinalysis and Ankle Brachial Index testing
• Collaborate with GPs and multidisciplinary teams to deliver coordinated, holistic care
• Identify eligible patients and support proactive patient recalls
• Maintain accurate clinical records and ensure services meet Medicare and ForHealth requirements
• Support other nurses and non-clinical team members in understanding CDM services
About You
• Current AHPRA registration as a Registered or Enrolled Nurse
• Previous Chronic Disease Management experience is highly regarded
• Primary healthcare or general practice experience is preferred
• Ideally, at least two years’ experience delivering direct clinical nursing care
• Sound understanding of health promotion and disease-prevention principles
• Strong written and verbal communication skills
• Ability to motivate patients and support them in taking ownership of their health
• Excellent organisation, prioritisation and time-management skills
• Proactive, adaptable and confident working independently
• Collaborative approach and commitment to delivering quality patient care
