Salary Range: $32.97 - $34.07 Hourly

Travel Percentage: Up to 50%
Job Shift: DayFamily Case Planner in a child welfare preventive services program to provide casework counseling to families who have children under age 18 at risk for placement outside the home. The program features the use of the Solution Based Casework model to partner with families in identifying the concerns and developing a service plan to address the concerns.
Primary Responsibilities and Essential Functions of Position:
- Direct Services to clients via casework, individual, family and group counseling as indicated by individual case assessments.
- Assist in Intake and Crisis Intervention
- Conduct home visits and attend client related meetings as needed.
- Develop client assessments and service plans following the model outlined by Improved Outcomes for children, Family Team Conference and SBC.
- Monitor and assist in service provision to all program clients as required by program mandates.
- Maintain current case records and service plans (FASP) for families served utilizing Connections and PROMIS systems.
- Participate in weekly supervision, case conferences and Family Team Conferences as directed by Supervisor and Program Director.
- Provide parent education and other support services utilizing group format as determined by client need.
- Participate in training appropriate to the development of enhanced Social Work skills as required by Children’s Services and HeartShare St Vincent’s Services.
- Assist in day to day operations as required by the Program Director.

Qualifications
- Completion of a Bachelor’s required; Masters degree preferred.
- At least two years of professional experience in Child Welfare or a closely related Human Services discipline preferred.
