As a Case Management Social Worker, you’ll serve as a key resource across multiple adult inpatient units, providing support wherever it is needed throughout the day. This float position offers the opportunity to work with diverse patient populations and collaborate with interdisciplinary teams across the hospital.
You’ll complete comprehensive psychosocial assessments, identify and address complex social barriers, and partner closely with assigned case managers and the medical team to support patient needs. You’ll also be cross-trained in discharge planning and may provide additional support in this area based on staffing needs.
A caregiver in this position works full-time, days, 8:00am—4:30pm. Rotating weekends and holidays.
