Job Shift: AnyJOB PURPOSE:
Ensure the delivery of services that support the prevention of homelessness by promoting program awareness, conducting comprehensive eligibility screenings, and connecting Veterans to appropriate resources and support services. Provide comprehensive, client-centered case management to Veterans and their families who are experiencing homelessness or are at risk of homelessness. Assist participants in achieving housing stability, increasing self-sufficiency, improving their quality of life, and maintaining the highest possible level of functioning within the community.

- - - - Advocate for Veterans and facilitate access to community resources and supportive services, including housing assistance, utility assistance, mental health services, medical care, and other available benefits. Develop and maintain a comprehensive database of community resources and provide appropriate referrals based on participant needs.
- - - - Conduct outreach and networking activities throughout the community, including visits to shelters, encampments, bus stations, stand-down events, and other locations where Veterans may be found. Deliver presentations to community organizations and stakeholders, actively participate in Veteran-focused community meetings, Coordinated Entry activities, and case conferencing meetings, and promote the mission, success, and reputation of the Supportive Services for Veteran Families (SSVF) Program.
- - - - Conduct initial screenings to determine eligibility for program services. Provide information and referrals to Veterans who do not meet eligibility requirements or when program capacity limitations prevent enrollment. Complete intake assessments, comprehensive needs evaluations, and individualized service plans that address housing stability and other identified needs. Coordinate, monitor, and document services to ensure progress toward established goals. Conduct housing inspections as required and maintain accurate, timely, and detailed case notes and client files in accordance with agency policies, procedures, and program requirements.
- - - - Regularly review and discuss assigned caseloads with the Site Supervisor to ensure effective service delivery and participant progress. Collaborate with supervisory staff to review outreach strategies, outreach logs, call logs, and program outcomes. Prepare reports as requested by the Program Director, Site Supervisor, or other leadership staff, and contribute to program evaluation efforts through recommendations for continuous improvement.
- - - - Assist Veterans in locating, securing, and maintaining safe, and affordable housing. Develop and maintain positive working relationships with landlords, property management companies, and community housing providers. Provide ongoing housing stabilization support to promote long-term housing retention and prevent homelessness or eviction.

ESSENTIAL QUALIFICATIONS:
EDUCATION: Bachelor’s Degree in Social Work, Sociology, Psychology or related field required.
EXPERIENCE: 1+ years case management experience; 3+ years preferred. 2+ years in a customer service focused environment. Experience with homeless, low income, veterans & their families a strong plus.
ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer’s sole discretion.
LICENSES: Driver’s License with clear record. LMSW, LBSW, LMFT preferred.
VEHICLE: Must have daily use of a vehicle without prior notice. Up to 50% travel within assigned counties.
OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must be available and willing to work nights, weekends and holidays as required to meet business needs. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.
