As a Case Manager you will work independently or be part of a team that co-ordinates a support network and links clients living with mental illnesses, addictions and physical health issues. The position attempts to empower the client, thereby increasing their independence, dignity and selfrespect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.
TAY (Transitional Aged Youth) Co-ops and Supportive Housing provides support to youth facing serious mental health challenges, who might also experience substance use challenges and/or homelessness. This program provides a range of supportive housing options across the city of Toronto Region either within LOFT owned houses or through head lease arrangements with private landlords. Each house is shared and comes fully furnished and with support services attached.
TAY Co-ops and Supportive Housing follows a client-centered approach and the psychosocial rehabilitation recovery model – a treatment approach designed to teach emotional, cognitive, and social skills that help those diagnosed with a mental illness live and work in their communities as independently as possible. Community Support Workers and clients meet frequently to design and revise an individualized service plan based on the client’s goals and aspirations. Mental health counselling and harm reduction strategies are often discussed and provided as well.
