Summary: The Case Management Supervisor (reporting to the Director of Social Services) has responsibility for the following:
ESSENTIAL DUTIES AND RESPONSIBILITIES: - Directly supervise the Case Managers - Provide orientation and initial training to new Case Managers and Social Work interns - Work with Clinical Supervisor, Assistant Director of Social Services and the Case Aide on quality assurance issues including closing of the charts - Review daily case notes in CARES and ensure that Case Managers input case notes within 24 hours - Monitor incident reports and ensure that follow up case notes are being attached - Approve documentation in all charts - Identify and coordinate the training and educational needs of the Case Management staff - Facilitate groups that will address the needs of the clients - Provide support to clients who are struggling to follow up on their ILPs - Cover caseloads when necessary - Document client engagement in CARES - Audit the clients’ charts to ensure compliance - Participate in case conferences, team meetings, staff training, community meetings, and staff meetings - Perform other duties as assigned. JOB QUALIFICATIONS: Educational Requirements: - Master’s Degree in Social Work (Licensed Master Social Worker). - Master's in Mental Health counseling or related field.
Experience: - Minimum of 3 years of clinical experience - Experience working with the mentally ill and/or homeless population - Flexible and comfortable in nontraditional setting - Excellent oral and written communication skills - Proven supervisory skills (in a supervisory capacity for a minimum of 3 years)
