The required shift for this position is: 8:00am- 4:30pm

We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.

The Specialized Treatment for Optimized Programming (STOP) Area Six Program connects California Department of Corrections and Rehabilitation inmates and parolees to comprehensive, evidence-based programming and services during their transition into the community, with priority given to those participants who are within their first year of release and who have been assessed as a moderate to high risk to reoffend. Area Six includes San Diego, Orange, and Imperial counties. STOP subcontracts with detoxification, licensed residential treatment programs, outpatient programs, professional services, and reentry and recovery housing throughout the program area to assist participants with reentry and recovery resources.

The Case Manager II works with a multidisciplinary team to provide casework services to clients, handle intake and initial placement in community-based services, transportation, case plan, case management, and interface with Division of Adult Parole Operations (DAPO) to assist clients in navigating the needed services to reach health and recovery. Case Manager II ensures that the quality of services delivered by the community-based providers (CBP) meet the standards set forth by the California Department of Corrections and Rehabilitation (CDCR) and HealthRIGHT 360.

KEY RESPONSIBILITIES
Case Management
• Maintain an assigned caseload of up to fifty (50) CDCR clients or more depending on needs.
• Provide learning experience opportunities and offer clinical support to assist clients in meeting their treatment goals.
• Develop Case Management Plans (CMP) for clients.
• Develop and monitor clients’ progress relative to their CMP through all of phases of the program and link them to both internal and external resources based on their treatment needs.
• Keep AOR apprised of clients’ progress and develops exit and aftercare plans.
• Travel to CBPs, sometimes requiring overnight stays, to provide outreach, case management, and assessment services.
• Correspond with AOR and assigns authorization for readmissions and client transfers.
• Assist with training of new hires.

Placement
• Assess, schedule, and coordinate placements of referrals into appropriate community placements.
• Coordinate transportation with the Transportation Coordinator.
• Screen paperwork for obstacles to treatment provision.
• Conduct daily communication with Parole Agents.
• Conduct presentations to community agencies to increase referrals.
• Identify community capacity needs.
• Revise and submit placement requests.

Documentation
• Maintain accurate and comprehensive files on each caseload client and community-based providers.
• Maintain progress notes in clients’ files.
• Complete reports as assigned.
• Maintain accurate records by entering data into various electronic systems.
• Help ensure the accuracy, confidentiality, and timeliness of incident reports.
• Reconcile Activity Reports with community providers.

Administration and Other Duties
• Ensure ongoing communication with the CDCR personnel regarding clients.
• Complete reports as assigned.
• Contact CBPs to request timely and sufficient reports.
• Report urgent incidents to supervisor.
• Attend required training and meetings
• Complete assigned training in a timely manner.

And perform other duties as assigned.

QUALIFICATIONS
Education, Certification, or Licensure
• Option I:
◦ High school diploma or equivalent.
◦ Valid Substance Use Disorder Counselor registration/certification from a certifying agency (CCAPP, CAADE, CADTP).
• Option II:
◦ Bachelor’s degree in Social Work, Psychology or related field.
• Valid First Aid and CPR certification or ability to obtain within 30 days of hire.
• Valid California Driver’s License and access to registered and insured transportation preferred.

Experience
• Minimum of two years’ case management experience with the criminal justice population.
