Job Description

Under the direction of the Program Director, the Case Manager is responsible for the delivery of quality family support case management services to clients in the areas of casework and referral to appropriate community resources. In this capacity, the Case Manager oversees and ensures a timely assessment of all individual client needs and collaborates with the family to develop appropriate individualized service plans for Family Support child care clients. Additionally, the Case Manager participates in the education, training and development of staff in regards to Family Support issues.

PROGRAM RESPONSIBILITIES:

  • Ensure a thorough intake into the program to include processing of new referrals, needs assessment, progress notes, service planning, casework summary, arrangement of transportation, and case closures.
  • Develop and support the implementation of program activities that are relevant to client needs.
  • Implement one-on-one and small group meetings for identified clients.
  • Compile accurate ongoing reports and bi-annual reviews.
  • Carry a caseload of at least 20-25 clients.
  • Act as a liaison with DCF, social workers and Y staff.
  • Report any suspected case of child abuse or neglect to the Program Director.
  • Communicate effectively and consistently with program director, DCF, Child Care Directors, staff and caregivers.
  • Attend all Family Support staff meetings and periodic program staff meetings.
  • Engage enthusiastically in trainings & events that support the promotion of protective factors among clients & families.

Qualifications

EDUCATION/EXPERIENCE/PHYSICAL REQUIREMENTS

  • Bachelor’s degree in a related field
  • Two years experience working with children preferred
  • Bilingual candidates strongly encouraged to apply
  • Minimum of 2 years’ experience in education, childcare, human services or social work settings.
  • Able to lift 30 pounds
  • Must be able to sit down, stand up, walk up and down stairs and bend over during the course of the day
  • Must provide proof of driver’s license, vehicle insurance and unattested driving record
  • Experience w/Microsoft office suite. Suite includes word, excel, access and PowerPoint.
