Job DetailsJob Location: Legal Address - La Crosse, WI 54603Position Type: Full TimeEducation Level: Bachelors DegreeJob Shift: DayBASIC PURPOSE:  Reporting to the Program Coordinator, the Case Manager provides services to clients in the sober living program. Services are typically provided in a residential setting.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Responsible for coordinating care and case plans for residents, participating as a member of the referral approval team, administering the on-site intake assessment and process, completing recovery plans, and monitoring progress. Responsible for completing resident discharge processes, addressing and correcting safety concerns and/or interpersonal issues among residents, and responding to crisis situations as necessary. Must enforce house rules, conduct drug/alcohol testing, and maintain a safe, drug-free environment.
Provide case management using a client-centered approach to residents. Work collaboratively with residents to meet resident needs, creating and maintaining a confidential environment, and assisting in coordinating educational and/or community services.  Responsible for assisting clients in securing employment, permanent housing, legal aid, and other community resources. Facilitating group services including peer support meetings, offer crisis intervention as needed.
Consulting and cooperating with internal programs and external partners to facilitate referrals and to assist in attaining the goals of the residents.
Measuring program outcomes by tracking program activities, monitoring participant progress, and maintaining organized records. Preparing, assisting, and completing time sensitive reports. This includes but is not limited to tracking resident enrollments, denials, and discharges. Maintaining detailed case notes, incident reports
Qualifications
ESSENTIAL KNOWLEDGE, SKILLS & ENVIRONMENT:
Education, Training; Work Experience: Bachelor or Master's degree in social work, psychology, or counseling is preferred. Experience/knowledge working with residential settings, adults with mental health and substance use disorders.  Maintain annual training hours required by FCC.
Specialized Knowledge & Skills: Trained in trauma informed care principles, Non-violent Crisis Intervention, skills-based interventions.
Equipment & Applications: Capable with Microsoft Office and Gmail/Calendar. Valid driver's license.
Work Environment & Physical Demands: General office setting conditions, to include work in a residential home setting. Some local travel by car is required. Ability to maintain CPI principles if needed in a crisis situation.
