About Us – Case Management
The Case Management team initiates discharge planning with every patient upon admission and provides assistance with aftercare, medical equipment, care coordination, and other discharge needs.
Case managers are assigned to individual units but also may follow patients with specific discharge needs to provide targeted, patient-centered care.
The Case Management team collaborates with physicians, nurses, ancillary staff, patients, and families to coordinate excellent care.
Case Managers are critical members of the multidisciplinary team and maintain consistent communication with care providers. The Case Management team strives to achieve timely discharge of patients with all needed aftercare arranged.
What You’ll Do
Requirements
Current Missouri Registered Nurse license or current Registered Nurse license from a compact state. If a compact license is held, it must be in the nurse’s state of residence.
or, Master of Social Work (MSW)
or, Licensed as an Respiratory Care Practitioner (RCP)
