Weekend Requirement: 1 weekend per 4-week schedule
Base Pay Range: $31.94 – $43.92 per hour (based on experience)
Shift Differentials:
Evening: +$2.50/hour
Weekend: +$2.00/hour
Combined Evening + Weekend: +$4.50/hour
Sign-On Bonus: $10,000 (paid in installments over 24 months)
Relocation Assistance: Available on a case-by-case basis
We are seeking an experienced Case Manager RN to support care coordination and utilization management within an acute care hospital setting in Tallahassee, Florida.
This role focuses on interdisciplinary collaboration, medical necessity review, discharge planning, and patient throughput. The ideal candidate brings recent acute care case management experience and strong knowledge of regulatory and payer requirements.
Conduct comprehensive psychosocial and clinical assessments of assigned patients.
Develop and coordinate individualized case management plans of care.
Lead interdisciplinary collaboration to ensure appropriate level of care and resource utilization.
Evaluate admissions and continued stays for medical necessity using approved criteria.
Perform utilization management reviews and communicate with third-party payers.
Facilitate timely and appropriate discharge planning.
Identify and address barriers to patient throughput and quality outcomes.
Coordinate referrals to community resources and post-acute services.
Document case management interventions and recommendations accurately.
Ensure compliance with regulatory standards and organizational policies.
Track trends and recommend process improvements to enhance efficiency and outcomes.
Required
Associate Degree in Nursing or Diploma in Nursing.
Active Florida RN license or compact license (Florida licensure required within designated timeframe if compact).
Minimum of 3 years of recent case management experience in an acute care setting.
Alternate Consideration
3 years of acute care experience in Med/Surg, Telemetry, Neuro, ICU, PCU, or ED.
Candidates with home health or insurance case management must have:
3 years total acute care experience, and
At least 1 year of acute care experience within the last 5 years.
Preferred
Bachelor’s Degree in Nursing.
Certification in Case Management, Nursing, or Utilization Review.
APRN license acceptable if current and compliant.
