This position has a unique emphasis on Pediatric Home Health services, with the goal of expanding pediatric care across much of CentraCare's Home Health and Hospice service area. The Case Manager in this role will help support the growth of our pediatric population and play an integral role in the development of a Pediatric Palliative Care and Hospice Program.
As we work to expand our pediatric footprint, this position requires flexibility to provide care throughout our full service area to care for both pediatric and adult patients. As our Pediatric Program continues to grow, this Case Manager will serve as a key clinical resource for Pediatric Home Health, Palliative Care and Hospice services for Pediatric patients.
Common responsibilities include comprehensive assessments, wound care, infusion therapy, care coordination and patient and family education related to new diagnoses and treatment plans. You'll receive extensive onboarding, ongoing education and dedicated RN office support while providing care in the field.

Based out of our office in Marketplace Mall in Waite Park, the Home Health department primarily takes place in the communities we serve, in the homes of our patients. Our staff provides services not just for St. Cloud but our surrounding areas as well including Buffalo, Alexandria, Little Falls and more.
