Summary/ Objective
East Side House (ESH) is a community resource in the Bronx and Northern Manhattan. ESH’s Adult & Workforce Development Programs aligns with our mission by providing high-quality programs and services within community centers and school partnerships.
Reporting to the Employment Coordinator, the Case Manager is responsible for a key role in the PSP staff inter-disciplinary team whose purpose is to provide comprehensive educational and emotional support that enable young adults to meet their personal, educational and employment goals. The Case Manager will support PSP participants in choosing a post-secondary track and completing outlined track deliverables for certification training, professional soft skills, internship hours, and job placement. As a member of the Adult & Workforce Development Programs, the Case Manager is expected to communicate effectively with the Assistant Director of PSP and Employment Coordinator to facilitate participant intake, case management and akll other wrap around services needed for participants who are enrolled in GED/ALE, PSP Academy, CRED and various other PSP service areas. This position’s direct reports include INDICATE WHO IS UNDER THE POSITION’S SUPERVISION and interacts with INDICATE WHICH OTHER DEPARTMENTS. With a focus on INDICATE WHAT AREA, the NAME POSITION provides INDICATE WHAT SERVICES/ OUTCOMES.

Expected Work Schedule
The work schedule expected for this position is: Monday through Friday, 9:00 a.m. to 5:00 p.m and possible weekend events, as aligned to the needs of the program/ service. The schedule may be adjusted based on ESH’s operational needs, with notice provided whenever possible.
