Founded in 2004, Brilliant Corners provides innovative housing and housing-related services to California’s most vulnerable individuals, with an emphasis on those transitioning from, or at risk of, homelessness or institutionalization. We develop, own, and manage multi-family supportive housing and licensed residential care homes. Department Summary
The Permanent Supportive Housing (PSH)-Project Based program is supported by Los Angeles County Homeless Services & Housing (HSH) and serves individuals experiencing homelessness who have a chronic illness or physical disability. The goal is to transition individuals out of homelessness and into permanent housing, in order to obtain stability in all areas of their lives. Services take place within residential communities for individuals who are experiencing homelessness and eligible for PSH at a project site.
The Case Manager, Permanent Supportive Housing Project Based (PSH-PB) will provide intensive case management services to a caseload of up to 20 homeless individuals who are eligible for permanent supportive housing at the project site located in the Central LA/Echo Park area. The Case Manager will assist clients to achieve and maintain health, mental health, and housing stability. The Case Manager plays a critical role in supporting clients to thrive in their new homes through a client-centered and “whatever it takes” approach to services.

A valid, clean CA driver’s license and a personal insured vehicle are required.
Travel Requirements
This position requires frequent visits to properties, attendance to on-site meetings, and other events in the greater Los Angeles County. Occasional attendance at events outside Los Angeles County, including Brilliant Corners’ events throughout California, may also be required.
Physical Requirements
Candidates should have physical mobility for tasks such as standing, bending, stooping, kneeling, crouching, reaching, twisting, and walking on uneven surfaces. They should be capable of performing stationary tasks like sitting for up to 6 to 8 hours a day. Additionally, candidates should be able to lift, carry, push, pull light to moderate weights up to 15 pounds safely. Requires mental acuity for analytical reasoning and document interpretation.
Salary range for this position is $25.48 to $28.49 annually. This position is being offered at $25.48 annually.
