PRN, Days
This Registered Nurse position is responsible for coordinating the patient needs with the Cooperative Care Team throughout the continuum of care by an interdisciplinary approach that integrates settings, services, health care practitioners, and care levels. The position does not involve direct patient care. Scheduling will require flexibility based on patient needs and coverage for other Cooperative Care Team members; this includes weekends and holidays.
Minimum Education, Training, and Experience
A. RN with at least 5 years of experience in acute care nursing required. BSN preferred.
B. Utilization review/discharge planning, case management experience preferred.
C. Current RN licensure to practice professional nursing in the Commonwealth of Kentucky. CCM preferred.
