Delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation in coordination with other health team members. Effectively interacts with the patient, patient family, and other health team members while maintaining high standards of professional nursing. Provides high quality care to the following patient populations: Adult patients experiencing a life limiting illness deemed to be in the last six months of life. Coordinates all hospice services required for optimal end of life care. Manages hospice resources for optimal patient care.The Case Manager works as a part of the interdisciplinary care team, addressing the patient's quality and plan of care, goals and expected outcomes, and readmissions prevention/reduction strategies. The case manager coordinates discharge education while serving as a resource to the care team with progressive work towards discharge planning in both the inpatient and Emergency Department settings.

Acute Inpatient Rehabilitation Department
1. Review potential admits with Clinical Liaison to assess for discharge planning needs/barriers.
2. Complete care management initial evaluation within two business days of admission.
3. Attend daily (M-F) rehab huddles as indicated by medical necessity and/or discharge planning.
4. Guide weekly required TEAMS meetings.
5. Complete Interdisciplinary Plan of Care (IPOC) power forms as indicated.
6. Complete concurrent insurance reviews.
7. Initiate post discharge follow-up phone calls on discharged patients within two business days.
8. Stay informed on Center for Medicare & Medicaid Services (CMS) regulations for acute inpatient rehabilitation.
