Colonial Behavioral Health provides services and support to individuals and families impacted by mental illness, substance use disorders, and developmental disabilities. We proudly serve the communities of James City County, the City of Poquoson, the City of Williamsburg, and York County.

We are currently seeking a Case Manager to join our Adult Mental Health department.

In this rewarding role, you’ll work directly with adults experiencing serious mental illness and/or substance use disorders. You’ll help individuals identify their goals, navigate challenges, connect with essential resources, and access the services and supports they need to thrive.

Examples of duties

Job Responsibilities

  • Develop an Individualized Service Plan (ISP)based on assessment information, in collaboration with the individual receiving services and, when appropriate, their family members or significant others.
  • Assist individuals directly in identifying and obtaining needed resources, services, and supports.
  • Monitor progress, service delivery, physical and mental health stability, and satisfaction for all individuals on the assigned caseload.
  • Coordinate services and supports to help individuals meet their identified goals and needs.
  • Maintain accurate and confidential documentation in accordance with agency policies and applicable regulations.
  • Perform other duties as assigned.

Qualifications

Monday – Friday, 8:30 a.m. – 5:00 p.m.

Skills and Qualifications

  • Knowledge of serious mental illness and co-occurring disorders.
  • Ability to maintain a high level of confidentiality and professionalism.
  • Strong communication, organization, and problem-solving skills.
  • Ability to assist individuals with problem-solving and use creative approaches when appropriate.
  • At least one year of experience working with individuals with serious mental illness and/or substance use disorders required.
  • Valid driver’s license is required.
  • Bachelor’s Degree in a human services field required.
  • Quality Mental Health Professional (QMHP) preferred.
