Job Description

New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, 24-hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee and St. Lucie counties. In addition, New Horizons assists 22,000 students in area schools, and we work closely with the courts, law enforcement, jails, and hospitals to help improve the health of individuals and the quality of life in our community.

We are seeking a Mental Health Court, CASE MANAGER, to provide screening, linkage, monitoring and substance abuse outreach services for adult clients under Mental Health Court supervision; overall responsibility relevant to coordinating treatment and services for Mental Health Court participants. Assist in obtaining community resources and benefits. Monitor, support and report progress to the court.

As a Mental Health Court, Case manager, your responsibilities would include:

  • Assess clients being referred to 19th Judicial Circuit Mental Health Court Project.
  • Completes comprehensive needs assessments to assigned clients in mental health court for purposes of identifying needs and services. This includes providing outreach services to help identify clients in need of substance abuse services.
  • Acts a primary Mental Health Court contact for New Horizons clients participating in Indian River Mental Health Court.
  • Coordinates with other New Horizons staff to assist clients within Mental Health Court to access needed services including mental health and substance abuse services
  • Provide assistance to clients for the purpose of accessing needed services
  • Participates in committees, meetings, clinical training, staff development and community relation activities.
  • Prepares written documentation for each service conducted.

Requirements

New Horizons will need you to have the following qualifications:

  • Bachelor's Degree from an accredited university or college with a major in the field of counseling, social work, psychology, nursing, rehabilitation, special education, health education, or a related human services field
  • One year experience
