Pay Range: $29.00 - $36.00 per hour DOE
Waterman Canyon Post Acute is seeking a compassionate, organized, and clinically skilled Licensed Vocational Nurse (LVN) Case Manager to join our interdisciplinary team. In this role, you will help coordinate resident care, support positive clinical outcomes, facilitate communication between healthcare providers, residents, and families, and assist with discharge planning and care transitions.
