Job Description

The Case Manager Opportunity:

The Case Manager provides community-based case management and rehabilitative support to individuals with mental health and/or substance use disorders. This role promotes recovery, independence, stability, safety, and community integration through assessment, service planning, skill development, crisis support, advocacy, and coordinated care. Case Managers partner with clients, families, treatment team members, and community providers to identify needs, strengths, goals, and barriers. Clients served may include adults or youth enrolled in Comprehensive Community Services (CCS), Community Support Program (CSP), Children’s Long-Term Support (CLTS), or the Coordinated Services Team (CST).

Compensation and Benefits:

  • The starting pay range for this role is $26.92-$31.70, based on education and experience.
  • This position is located in Lincoln County and works with youth in the CCS and CLTS programs.
  • This job is full-time and in person, generally working 8:00am-4:30pm on weekdays.

  • As a full-time employee of NCHC, you are eligible for the Wisconsin Retirement System (WRS). The WRS is a hybrid defined benefit plan. It contains elements of both a 401(k) or defined contribution plan and a defined benefit plan. Participating staff contribute 7.2% and NCHC will match this at 100% each pay period!

What Qualifies You:

Required:

  • Bachelor’s degree in Human Services, Social Work, or a closely related field.
  • More than 1 year of experience in case management.
  • Must possess a valid Wisconsin driver's license and maintain unrestricted driving privileges throughout employment and have use of a personal vehicle.
  • Must meet and continue to meet all requirements for insurability under the organization's vehicle insurance policy.
  • Must be authorized to operate any vehicle and transport clients, patients, or staff as required by the position.

Preferred:

  • Master’s degree in Human Services, Social Work, or a closely related field.
  • More than 3 years of experience in case management.
  • More than 1 year of experience in CCS, CSP, CLTS or CST

What a Case Manager does at NCHC:

  • Conduct initial and ongoing assessments to identify mental health, substance use, functional, psychosocial, educational, and environmental needs.
  • Facilitate treatment and service planning, including identifying strengths, needs, goals, objectives, interventions, barriers, and priorities.
  • Determine program eligibility through state-approved functional screens.

  • Complete treatment plan reviews, updates, and revisions to support ongoing progress and stability.
  • Coordinate services across multiple systems such as psychiatry, primary care, schools, courts, housing, AODA providers, crisis, and contracted services.
  • Organize and facilitate multidisciplinary team meetings with clients, families, and providers to ensure cohesive care.
  • Advocate for client needs to obtain medical, dental, legal, housing, financial, and community resources.
  • Monitor progress and adjust interventions as needed to address symptoms, functioning, safety, or emerging needs.
  • Provide side-by-side skill teaching in daily living areas such as cleaning, cooking, money management, hygiene, transportation, and community navigation.
  • Use motivational interviewing and rehabilitative strategies to enhance coping, problem solving, social skills, symptom management, and independent living abilities.
  • Support individuals in building natural supports and community connections.
  • Assess symptoms, risk factors, and crisis situations; coordinate with crisis professionals to ensure safety and appropriate response.
  • Assist in developing individualized crisis plans and collaborate with crisis services during emergent situations.
  • Support stabilization through safety planning, referral to emergency services, and ongoing follow-up.
  • Complete documentation that meets program standards, state regulations, and agency policy, including progress notes, assessments, crisis plans, referrals, functional screens, and treatment plans.
  • Communicate effectively through written and electronic methods with team members, NCHC departments, and community partners.
  • Assist clients in developing the skills necessary to seek, obtain, and maintain employment, including hygiene, punctuality, organization, job-related supports, and transportation planning.
  • Provide structured interventions to improve social functioning and engagement in recreational and community activities.
  • Conduct hospital or facility visits to support discharge planning, continuity of care, and reintegration into the community.
  • Coordinate and monitor transitions between levels of care.
  • Perform other duties as assigned in support of program needs.
