As a Case Manager in our reentry program, you’ll build supportive relationships with participants, help them navigate resources, and work alongside them as they set and pursue goals. You’ll support program participation and help people take practical steps toward long-term, stable housing.
$22.71 per hour. This full-time position is eligible for medical, dental, and vision coverage, paid time off (PTO), Holidays, and 401(k). May require occasional early mornings, evenings, or weekends as needed.

· Build rapport with participants and learn about their needs through conversations, records review, and consultation.
· Meet with assigned participants monthly to develop and update Individual Reentry Plans (IRPs), set goals, review progress, and identify next steps.
· Support participants in understanding and meeting program requirements.
· Help participants access community resources and services, which may include housing, healthcare, SNAP, DMV appointments, employment supports, and other services.
· Coordinate with professionals and community partners involved in participants’ success, such as healthcare providers, counselors, legal supports, DHS workers, sponsors, family members, and parole officers.
· Respond to crises using trauma-informed de-escalation approaches and help connect participants with appropriate, timely resources.
· Develop and maintain a network of community resources, including medical and mental health services, DUI classes, social supports, financial coaching, and legal assistance.
· Build relationships with landlords, property managers, housing authorities, and affordable-housing partners; stay informed about housing opportunities and relevant requirements.
· Provide practical education and resources to help participants secure and maintain permanent housing, including coordinating rental assistance applications and payments.
· Maintain accurate documentation and protect participant confidentiality.
· High school diploma or equivalent.
· At least one year of case management or social services experience.
· Strong interpersonal, verbal, and written communication skills.
· Careful documentation and basic use of technology skills, including email, Microsoft Word and Excel, and data entry.
· A respectful, person-centered and trauma-informed approach and the ability to work collaboratively with participants and community partners.
· Direct or lived experience related to homelessness, addiction, and/or working with low-income populations.
· Understanding of issues that may affect participants, including homelessness, chemical dependency, domestic violence, and sexual assault.
Work is primarily indoors in an office setting, with occasional moderate noise and exposure to scents. The role frequently involves sitting or standing at a computer and using standard office equipment, with occasional movement throughout the facility for meetings and work needs.

Helping Hands Reentry Outreach Centers was founded by Alan Evans, who was homeless for over 25 years. He was removed from an abusive home and sent to foster care at age 11, but when his circumstances didn't improve, he ran away and was homeless and addicted by age 13. Without adequate resources to address his childhood traumas and help him make positive changes to be able to reenter society, getting arrested meant getting rescued. But his arresting officer asked to hear his story and helped him find sobriety and sustainability. Evans started Helping Hands to give other homeless people what he himself had needed: a helping hand to a sustainable life.
In 2002, Evans opened an 8-bed home for the homeless in Seaside, OR, and over the years Helping Hands has expanded to meet the changing needs for homeless services in surrounding communities. When more homeless seniors and families with children began needing Helping Hands' support, we began to offer Case Management with Individual Reentry Plans to guide each person in gaining skills and accessing the resources they need to rebuild self-sufficiency. With trauma-informed best practices, we collect our clients' stories, demographics, and needs in our proprietary database and use this to guide their treatment. We now operate 11 emergency homeless shelter and Reentry Program facilities, providing over 350 beds per night in Clatsop, Tillamook, Yamhill, Multnomah, & Lincoln Counties in Oregon. The most recent project to expand capacity to serve the homeless has been the Bybee Lakes Hope Center at the former Wapato Jail property in the Port of Portland, OR.
