The Opportunity

We are seeking a Case Manager – LEAD to support individuals enrolled in the Law Enforcement Assisted Diversion (LEAD) program by coordinating services across systems of care and serving as a primary point of engagement for participants and law enforcement partners. In this role, you will assess needs across behavioral health, medical, housing, and social service domains; facilitate referrals to internal and community-based providers; and actively support participants in progressing toward individualized goals. You will work closely with participants, peer recovery staff, law enforcement, and community stakeholders to promote recovery, stability, and successful community integration while preventing involvement with the criminal justice system. This position requires a person-centered, trauma-informed approach and strong collaboration within a multidisciplinary team to ensure high-quality, coordinated care.

What You’ll Do

• Provide services in the least restrictive environment, case managers will be required to “meet LEAD participants where they are” meeting with them in their home or other identified community setting

• Maintain a case load of LEAD participant clients and provide specialized intensive case management services as needed

• Work closely with Police partners who are the primary referral source for LEAD

• Provide linkages to care and other community resources and supports to assist participants in maintaining recovery and preventing entrance into the criminal justice system

• Work with the LEAD Peer Recovery Specialist to help participants navigate and overcome barriers in accessing needed health and behavioral health care

• Maintain 10-6pm schedule with the ability to be flexible with hours when necessary, depending on the needs of participants being served

• Maintain collaborative relationships with community partners and support service agencies.

• Provide transportation to participants for appointments when necessary

• Complete timely and accurate documentation

• Complete outcome tracking forms for caseload of participants

• Attend weekly, operational workgroup meetings to discuss the progress of LEAD participants

• Attend trainings and conferences as needed to improve program skills

• Additional responsibilities as determined by the LEAD Program Manager

Qualifications

Education:

  • This position requires a Bachelor’s degree in social work, psychology or a related human service field from an accredited college or university.



Experience:

  • Experience working with individuals with mental illness, addiction and/or co-occurring disorders is given priority.
