Job Description

The substance use case manager will provide comprehensive case management services to individuals with substance use disorders. This position is for the evening expanded hours program, evenings are required. This role will focus on assessing client needs, developing individualized service plans, coordinating treatment, and support services. This will help promote long-term recovery. The case manager will work collaboratively with the senior counselor, client, families, treatment providers, and community resources to ensure continuity of care and support harm reduction, recovery oriented goals.

This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to teamwork and cooperation.

QUALIFICATIONS

EDUCATION: Bachelor's degree in related field required.

LICENSE/CERTIFICATION: CADC preferred. . Valid Driver's License required. Annual, American Heart Association BLS and Heart Saver First Aid required within 90 days of hire or transfer. CPI required within 90 days of hire or transfer.

BACKGROUND CHECK Ability to pass a background check through fingerprint process and drug screen as required.

EXPERIENCE: Previous case management experience required.

PERFORMANCE EXPECTATIONS

The case manager will work closely with clinical staff, peer support specialists, health care providers, courts, probation, and community partners. They will participate in the multidisciplinary team meetings and case conferencing and will advocate for client needs across system and help reduce barriers to treatment.


  • DUTIES AND RESPONSIBILITIESConduct initial screenings and ongoing needs
  • Develop, implement, and monitor individualized treatment and recovery service
  • Provides crisis intervention, individual counseling, and strength based
  • Facilitate referrals for mental health, housing, employment, court, and other supportive services.
  • Monitor client progress, provide follow-up, and adjust service plans as
  • Coordinate or provide transportation for clients when
  • Maintain accurate timely confidential documentation in compliance with HIPPAA state regulations and AtlantiCare addiction services policies.
  • Ensure compliance with grant requirements, program standards, and funding

WORK ENVIRONMENT

This position requires the case manager to work in the following environments: office, community, and field-based work. May require occasional evening or weekend hours depending on program needs.This position requires repetitive motion, hearing, pushing, reaching, and reading, seeing, speaking, standing and walking a majority of the time. Spends time with such equipment as computer terminal, fax machine, copier, and telephone. Occasional driving also required.
