Travel Percentage Road Warrior Job Shift Day

The Case Manager provides transitional case management support and other various clinical services as needed to individuals in custody or recently incarcerated. This position will interact with corrections staff and clients to coordinate care and support community reintegration. Services may be provided in a jail detention setting, in the community, or in one of the program locations so the Case Manager will travel regularly throughout north central Washington.

Responsibilities:

  • Provides quality case management services to clients with a behavioral health disorder who are in the criminal justice system within Chelan, Douglas, and Grant Counties
  • Conducts jail services intake screenings
  • Completes progress notes and other required documentation in the electronic consumer files within the required timeframe.
  • Services may include referral to internal crisis teams, assisting with applying for benefits, activities of daily living, psychosocial support, service coordination and linkage, transportation, referrals for medication management and behavioral health counseling, and assistance accessing community resources and housing/
  • Establishes and maintains working relationships with community resources, coordinates consumer treatment with other providers, and initiates referrals to medical providers and other community resources when appropriate.
  • May attend community partner meetings in support of care coordination
  • Performs other duties as assigned

Job Requirements:

Physical Requirements:

This position normally requires the physical demands of standing, walking, bending, lifting or performing other work requiring low physical exertion, talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time. Position also requires ability to drive with adequate vision and skill. This position entails extensive driving within the Catholic Charities service area.

Non-Physical Requirements:

Education

  • Associate degree in behavioral health, social sciences, or criminal justice required

Experience

  • 2 years of experience in case management or behavioral health treatment preferred

Special Skills

  • Possess and utilize effective verbal and written communication skills
  • Ability to be self-motivated, work independently and as a team member
  • Bilingual (English/Spanish) preferred but not required

Licensure, Registration, Certification

  • Valid Washington State driver’s license and minimum required liability insurance for WA State
  • Must be deemed insurable as determined by Catholic Charities’ liability insurance provider
  • Agency Affiliated Counselor Registration

Employment is conditional upon:

  • Being cleared by criminal background check and fingerprinting when required

Work Schedule: Monday to Friday, 8:00am- 5:00pm
Wage Range: $23.32 - $28.62 per hour, depending on experience
