At The Salvation Army, we believe everyone deserves a safe place to call home. We're looking for a passionate and driven ICMI Case Manager to join our Homelessness East team and make a meaningful difference in the lives of people experiencing or at risk of homelessness.
In this rewarding outreach-based role, you'll provide intensive, person-centred support to individuals with complex needs, helping them secure and sustain safe, affordable housing while reducing their reliance on the homelessness service system. Working alongside clients, you'll empower them to achieve their goals, strengthen community connections, and create lasting positive change.
As a Case Manager in Intensive Case Management Initiative (ICMI), you'll work closely with clients through outreach and intensive support sessions, delivering holistic case management that responds to their unique circumstances, vulnerabilities, and aspirations.
You'll develop tailored case and safety plans, coordinate access to specialist services, advocate on behalf of clients, and provide practical support to help people achieve stable housing outcomes.
Reporting to the Team Leader, this is a permanent full-time opportunity, based on-site in Leongatha, VIC.
Compensation is in accordance with the SCHADS Award Level 5.
