The Case Manager II plays an important role in helping individuals successfully navigate treatment and prepare for life beyond our program. This position works closely with patients and the multidisciplinary treatment team to identify barriers, coordinate services, connect patients with community resources, and build a strong foundation for continued recovery.

What You’ll Do

The Case Manager II provides comprehensive case management services for individuals with behavioral health and substance use needs.

Key responsibilities include:

  • Conduct biopsychosocial assessments and contribute to treatment plan development and updates
  • Coordinate care among clinical, medical, and recovery support teams to promote continuity of services
  • Provide case management for patients with moderate to high needs
  • Assist with crisis coordination and complex psychosocial needs
  • Coordinate discharge planning and help patients prepare for the next level of care
  • Facilitate and participate in interdisciplinary case reviews
  • Communicate patient progress, barriers, resource needs, and next steps with the treatment team
  • Establish and maintain collaborative relationships with community agencies, courts, referral sources, and other providers
  • Help patients access resources related to:
    • Housing
    • Employment
    • Transportation
    • Insurance and public benefits
    • Medical and behavioral healthcare
    • Recovery support services
    • Community-based programs
  • Advocate for patients while encouraging independence and active participation in recovery
  • Maintain timely, accurate, and clinically appropriate documentation in the electronic health record (EHR)
  • Participate in performance improvement and compliance initiatives
  • Maintain appropriate professional boundaries and patient confidentiality
  • Perform other duties as assigned

Who You Are

You understand that recovery involves much more than what happens during counseling or treatment programming.

You are someone who can build trust while maintaining professional boundaries, coordinate multiple moving pieces, communicate effectively across disciplines, and consistently follow through.

You are comfortable working with individuals who may be experiencing significant stress, substance use challenges, behavioral health symptoms, housing instability, or other barriers to successful recovery.

Most importantly, you approach patients with dignity, empathy, accountability, and respect.

Required Qualifications

Candidates must meet one of the following education/experience pathways:

  • Bachelor’s degree in social work, psychology, counseling, or a related field; or
  • At least three (3) years of behavioral health experience with demonstrated case management competencies

Additional requirements include:

  • Working knowledge of ASAM Criteria, Stages of Change, and recovery-oriented principles
  • Proficiency in electronic health record documentation
  • Strong organization and time-management skills
  • Effective verbal and written communication skills
  • Ability to collaborate within an interdisciplinary treatment team
  • Current CPR/First Aid certification or ability to obtain within 30 days of hire

Preferred Qualifications

Preferred candidates may have:

  • Master’s degree in a behavioral health or related field
  • Clinical licensure such as LCSW, LMHC, LMFT, LCAC, or another applicable credential
  • Two or more years of experience in residential or outpatient substance use disorder or co-occurring treatment
  • Training in motivational interviewing
  • Experience with trauma-informed care
  • CADAC or similar substance use disorder certification, as applicable

This position requires the ability to:

  • Sit, stand, and walk for extended periods
  • Occasionally lift or move up to 25 pounds
  • Effectively observe and respond to patient needs
  • Communicate clearly with patients, staff, and community partners
  • Concentrate, problem-solve, and manage competing priorities in a fast-paced environment
  • Maintain professionalism, empathy, and composure during crisis situations or when supporting patients with complex needs
