For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness.
Older Adult Centers promote an active life for seniors. The centers aim to assist them to preserve their independence and health by offering various range of activities and services such as serving breakfast, lunch, health screenings, social services, educational workshops, class exercises, discussion groups, games, art classes, lectures, workshops, organized outings, and special events.
STATEMENT OF THE JOB
Under the direction of the Program Manager, the Case Manager II determines the client needs for assistance, arranges for services, provides information and referral and assists the Program Manager to plan and coordinate education/recreational activities for the Program.
• Assess needs and provide information, referral, and case assistance to seniors.
• Develop an appropriate care plan and follow through to disposition.
• Maintain records, case files, and statistics regarding client files and activities according to DFTA and agency regulations.
• Lead a group discussion at least biweekly.
• Provide monthly benefits information workshops for seniors.
• Coordinate special events and activities as assigned by the Program Manager.
QUALIFICATIONS:
• Bachelor’s Degree in human-related services from an accredited college or university.
• Knowledge of entitlement for older adults preferred.
• Proficient in computer software applications, Excel, and Microsoft Office.
• Bilingual preferred.
• Able to travel to client’s homes, meetings, conferences, and other locations for training.
• Ability to lift to 10lbs.
