Salary Range: $20.75 - $22.00 Hourly

Case Manager I - Targeted Case Management (Metro County)
People Incorporated Mental Health Services is seeking Case Manager I to work on our Targeted Case Management (Metro County) team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules.
Hours: Full-Time, Monday- Friday, business hours 8am-4:30pm
Location: Metro County, Minnesota (Hybrid)
Hiring Range: $20.75 - $22.00 hour
Hiring Bonus: $1000 to external candidates

Main Job Duties:
- Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
- Provide general knowledge and guidance on assisting clients in the multiple phases of applying for and receiving services and benefits.
- Attending bi-annual meetings with clients to monitor services
- Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications
- Assessing client needs and ensuring continuity of care. This may include outreach and consultation with other partners and providers.
- Maintain regular contact with collaborative partners and make referrals to professionals, schools and community agencies, as needed
- Making referrals and authorizing services based on needs
- Maintaining records and documenting all coordination activities
- Excels at documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards

QualificationsEducation and

Experience:
- Bachelors Degree in the behavioral sciences or related field (including but not limited to social work, psychology, or nursing from an accredited college or university)
- If no Bachelors Degree in a behavioral science or related field, the employee must meet one of the following:
- Have 3 or 4 years experience as a case manager aide; OR
- Be an RN without a Bachelors Degree and have 3 years or more combined specialized training in psychiatry and work experience consisting of community interaction and involvement or community discharge planning in a mental health setting.
