People Incorporated is seeking a Case Manager I or II to join our Targeted Case Management (Dakota County) team to primarily work with clients who are diagnosed with chronic mental illness or other chronic health conditions. We connect people with services through developing person-centered support plans that help them live as independently as possible. Our Case Managers develop long term relationships with their clients and operate autonomously, creating their own schedules.
Hours: Full - Time, Monday - Friday, business hours
Location: Dakota County, Minnesota l Hybrid
Case Manager I - $20.75 - $22.00 Hourly, based on qualifications
Case Manager II - $24.25 - $25.75 Hourly, based on qualifications
Hiring Bonus: $1,000 to external candidates
