The Opportunity

Berry Street Yooralla's Housing Services team sits within the Specialist Care and Transitions North West team within the wider Complex Care program. The team provides multidisciplinary, trauma-informed support to young people and families experiencing vulnerability, disengagement and housing instability.

The Case Manager, Housing Services will provide a case management response to clients with or without accompanying children who are experiencing homelessness or at risk of becoming homeless. Clients who access these supports are often in crisis and have multiple, complex needs making them vulnerable and as such the program aims to identify and address the issues that have led to their current situation. The program focuses on principles of social inclusion and social justice to ensure that those who have been overlooked by every other element of the system have a place to go.

In this role, you will:

  • Conduct client assessments and develop client-led case plans that support stability, independence and improved outcomes
  • Identify and respond to the needs of children, young people and families, ensuring inclusive and tailored support approaches
  • Connect clients with housing, accommodation and broader service options to address homelessness and promote long-term stability
  • Build and maintain strong partnerships with community and social service providers to enable effective referrals and collaboration
  • Advocate for clients and apply early intervention strategies to improve access to services and strengthen outcomes across the Western region.

About You

We are looking for a passionate and values-driven practitioner who is committed to supporting people experiencing housing instability. You will bring a client-centred, strengths-based approach and thrive in collaborative, community-focused environments.

  • Experience in case management within homelessness, housing, or community services
  • Ability to engage respectfully with children, young people and families, and respond to diverse needs
  • Strong understanding of trauma-informed practice, early intervention and client-led planning
  • Well-developed skills in building partnerships and working collaboratively with external services
  • Confidence in advocating for clients and navigating service systems to achieve positive outcomes
  • You will hold a Bachelor degree in Social Work, Education, Psychology or a related human services field.

We are the major specialist paediatric hospital in Victoria and our care extends to children from Tasmania, southern New South Wales and other states around Australia and overseas.
