About Us
Avivo’s Ending Homelessness division helps individuals and families move from homelessness to housing stability. We partner with county coordinated entry systems, tribal governments, and operate unique street outreach teams to connect people with shelter and permanent housing. We offer support in both scattered-site and site-based housing, including transitional recovery housing and Avivo Village—a low-barrier, culturally responsive shelter designed to support unsheltered individuals on their path to stable housing. Our work is grounded in harm reduction, housing first, and trauma-informed care with a commitment to culturally responsive, person-centered services.
Your Role
As a Case Manager in our scattered site housing programs, you’ll provide one-on-one support to individuals and families referred from Hennepin County shelters. You’ll help clients set goals, access community resources, and prepare for long-term housing success. Your role includes assisting clients in shelters and in the community as they transition into stable housing and beyond.
Schedule: This is a 32/hr. a week full-time position paid for 40 hours.
