Role: Case Manager, FSP

Report to: Program Manager

Program: 1303-FSP

Department: Mental Health

Location: Wellness Center - 345 S. San Pedro Street, Los Angeles, CA 90013

Schedule: Monday - Friday, 8am - 4:30pm

Status: Full-Time, Non-Exempt (hourly), Non-Management

Summary

Full Service Partnership is a specialized mental health program that provides wraparound and individualized field-based services to individuals with complex needs, including serious mental illness, chronic health issues, substance use disorders and homelessness.

The Senior Case Manager is responsible for leading a collaborative effort to house the most vulnerable and chronically homeless individuals in SPAs 4 and 5. With the support of and in partnership with the rest of the FSP team, the Senior Case Manager will ensure that FSP clients have a successful process for obtaining housing and accessing other supportive services.

Essential Duties and Responsibilities

  • Provide linkage to stable housing, and other supportive services as needed to assist clients to obtain housing stability, in addition to client advocacy, case management, and benefit establishment
  • Identify appropriate permanent housing options for clients and support them through the process such as by conducting screening interviews, completing housing documentation, and coordinating clients’ move-in with the housing authority and property managers
  • Perform initial comprehensive case management assessment and develop a plan of intervention that addresses their barriers, increases their income, and assists them in maintaining permanent housing
  • Encourage and promote an environment that is strength based to assist clients in meeting their individual goals
  • Complete all documentation and maintain productivity expectations for billable time, according to standards and as they are required by our funder (the Department of Mental Health)
  • Actively participate in staff meetings and trainings
  • Assist in crisis interventions as needed, including participating in on call rotation calendar for overnight and weekend After Hours Emergency Line

Qualifications

  • Bachelor’s degree in related field, or an equivalent combination of education and experience
  • Three (3) years or more of Case Management experience
  • At least one (1) year of experience working with adults with severe, persistent mental illness and co-ocurring substance use disorders, and with chronically homeless individuals
  • Current, valid CA Driver’s license, reliable personal vehicle with auto insurance, and willingness to transport clients in personal vehicle
  • Experience and familiarity with LA County resources, benefits, and services to help clients meet their case management needs
  • Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills
  • Strong interpersonal and team building skills
  • Knowledge of Public Housing Authority and Housings subsidy preferred (Section 8, Shelter Plus Care, VASH)

Preferred Qualifications

  • Bilingual Spanish

Work Environment

  • Combination of field and office environment
  • Regularly required to sit, stand, bend; occasionally lift or carry up to 35 lbs.
  • On occasion walk or drive to different local sites
  • Will necessitate working in busy and loud environments
  • Will be exposed to elements like cold, heat, dust, noise and odor
  • May need to bend, stoop, twist, and sit throughout the day
