Job Description

Job Purpose or Objective(s): As the Case Manager, you will be a source of social, educational, and spiritual support for individuals and families to develop skills and enhance quality of life. You will report to the Departmental Manager or Director.

Primary Tasks:

  • You will contact clients to perform intake assessment and analysis.
  • Provide an overview to the client by describing the extent and nature of the services available to them.
  • Provide educational and social case management services to clients including setting goals and developing case plans
  • Review and monitor participant progress and provide resources, advocacy, and referrals.
  • You will manage case to ensure progress and adjust as needs change.
  • Develop working relationships with individual patient and family.
  • Make referrals to community agencies and conduct follow-up activities.
  • Be a patient liaison and advocate.
  • Perform other responsibilities as assigned.

Job Requirements:

  • Bachelor’s degree in human service, social work, or education field and one [1] year directly related experience, OR five [5] years of directly related experience in lieu of education.
  • Good social skills and ability to build trust and relationships.
  • Good oral and written communication skills
  • Basic knowledge of Microsoft Word and Excel
