OUR VALUES
The goal of the Housing Retention Case Management program is to provide individuals transitioning from homelessness to permanent supportive housing with access to resources and comprehensive support. These case management services are designed to empower participants to manage their daily lives more effectively, while promoting self-sufficiency to maintain stable housing. This role will also provide services to our North County, Markham Plaza, Iamesi Village, EHV, Mesa Terrace and Solaire Apt locations within Santa Clara County.
REPORTING RELATIONSHIPS:
This role reports to the Program Manager.
