A Qualified Professional (10A NCAC 27G .0104) must have one of the following:
Bachelor degree (non Human Services) with 4 years Full time OR 8 years Part time applicable experience required
Bachelor degree (Human Services) with 2 years Full time OR 4 years Part time applicable experience required
Master degree (Human Services) with 1 year Full time OR 2 years Part time applicable experience requiredThe primary responsibilities of the Case Manager, Child Crisis are to plan, coordinate, track and monitor care across a spectrum of services for Monarch’s Crisis & Urgent Care Services supporting children/adolescents with complex multi-system involvement and/or chronic physical health conditions. This includes care coordination specifically from discharge to recommended services.
