Salary Range: $24.00 - $26.00 Hourly

Travel Percentage: Up to 25%As a Case Manager in the Clinical Time Transition Program (CTTP), you will support youth and their caregivers enrolled in the Northern Rivers Hospital to Home Program. You will perform tasks in a phase-based Critical Time Intervention (CTI) model of care. You will report to the Director of the Hospital to Home program and you will be working onsite with community travel.
Responsibilities:
- Manage a caseload of clients in both home and Transitional Residential Setting (TRS) through all phases of the time-limited CTI model.
- Collaborate with referring to hospital programs.
- Outreach to referred clients for ongoing engagement.
- Establish treatment plan including goals, phase transitions, and discharge planning.
- Coordinate comprehensive evaluation and connection to necessary mental health treatments.
- Evaluate needs, refer, and connect youth and families to community support.
- Establish necessary social care need connections.
- Help address problems in access to and delivery of, services through the creation of relationships between the clients and community resources.
- Transport to and from appointments.
- Collaboration with schools to support youth’s educational needs.
- Train and enhance youth and family’s community living skills.
- Manage transfer of caregiving to services and supports in the community to establish continuity of care post discharge.
- Complete accurate case notes and input the notes into required internal and external systems (Electronic Health Records)
- Access and use data from Regional Health Information Organization (RHIOS)/Qualified Entities (QEs), PSYCKES, and other data systems as part of their work.
- Be a contributing member of the interdisciplinary CTTP treatment team
- Coordinate and attend all applicable meetings about caseload.

Requirements and Education:
- Bachelor's degree required
- 1+ years experience in case management and mental health populations and systems preferred.
- Valid NYS driver's license required.
