Case Manager, Clinical Stabilization Services (CSS)

  • Hours 7am - 3:30pm from Friday to Monday. You must be able to work this schedule every week
  • Pay $29/hr - $38/hr dependent on related experience, education and licensure

Objective

The Case Manager is a key member of the Clinical Stabilization Services (CSS) treatment team and provides care coordination and discharge planning for individuals receiving residential substance use treatment. This role works closely with clinicians, recovery specialists, medical providers, and community partners to help clients access services, address barriers, and successfully transition back into the community.

Essential Functions

  • Meet regularly with clients to provide care coordination and aftercare planning.
  • Conduct assessments to identify client needs, strengths, and barriers to treatment goals.
  • Develop care plans in collaboration with clients and the treatment team.
  • Coordinate referrals and linkages to services including housing, employment, healthcare, and financial supports.
  • Coordinate care with outside providers such as courts, primary care providers, and behavioral health specialists.
  • Facilitate psychoeducational groups as assigned.
  • Provide transportation to appointments or community services when needed.
  • Plan and coordinate discharge and aftercare services to ensure continuity of care.
  • Conduct post-discharge follow-up with clients.
  • Assist the clinical team with crisis management as needed.
  • Maintain professional boundaries and respectful relationships with clients.
  • Complete documentation in a timely manner in accordance with Joint Commission standards, organizational policies, and applicable state and federal regulations.
  • Attend required staff meetings and trainings and perform other duties as assigned.

Intermediate computer proficiency including Microsoft Word, Excel, and electronic documentation systems.

Work Environment

This position operates primarily in a clinical residential treatment setting and may require transporting clients to community appointments.

Physical Demands

Light physical activity, including lifting or carrying up to 20 pounds.

Required Education

  • Bachelor’s degree in Human Services, Psychology, Sociology, Health Services, or a related field. A Masters Degree or current enrollment in a MSW program or similar is highly preferred but not required. This role is ideal if you are looking for relevant experience while finishing your graduate studies
  • At least one year of experience working in a substance use treatment program.

Preferred Education/Experience

  • Two or more years of experience working with individuals experiencing substance use disorders, mental illness, or homelessness.
