Job Category: Nonprofit - Social ServicesJoin our team as a Case Manager with our NEW Coordinated Specialty Care (CSC) program in La Crosse, WI!
BASIC PURPOSE: To provide case manager services to young person’s ages 15 - 25 years, with early onset psychosis in the CSC program, which allows them to live successfully in the community and grow their recovery.

ESSENTIAL DUTIES & RESPONSIBILITIES:
- Assist in development of CSC program processes in accordance with grant requirements.
- Assist in development and maintenance of relationships with external providers to coordinate resources and referrals.
- Provide assessment, treatment planning, education and assistance in developing and completing activities of daily living, recreational and social skills, providing symptom monitoring / management and education of symptom management tools, as well as crisis intervention.
- Engage and coordinate with family/support systems.
- Attend all team and client-related treatment meetings.
- Coordinate care for clients which may include: psychotherapy, skills training, financial counseling, housing assistance, substance abuse counseling and treatment, and family counseling.
- Ensure all client service related paperwork is completed and in a timely manner consistent with policies and procedures for the CSC program.

QualificationsESSENTIAL KNOWLEDGE, SKILLS & ENVIRONMENT:
Education, Training & Work

Experience: Preferred Master’s degree in social work, clinical psychology or psychiatric mental health nursing, occupational therapy, or having met equivalent educational requirements with 3,000 hours of supervised clinical experience working with persons with severe and persistent mental illness.
Specialized Knowledge & Skills: CBT, family education and support, and substance use disorders. Knowledge of clozapine for medication resistant symptoms.
Equipment & Applications:  Ability to use Microsoft Office programs, e-mail, scanner, and FAX machine, as well as enterprise software. Driver’s license and ability to use own vehicle to provide client transportation

Work Environment & Physical Demands: Indoor office environment, as well as at least 50% of work in a community setting or in client’s homes. Local travel will be required. Additional physical demands could include occasional lifting, bending, twisting, or reaching
